Der Alto Alentejo ist eine portugiesische Subregion im Nordosten der Region Alentejo.
Im Jahr 2021 verzeichnete die Subregion 104.989 Einwohner und eine Bevölkerungsdichte von 17 Einwohnern pro km2. Die Subregion hat eine Fläche von 6.084 km2, welche sich in 15 Kreise und 69 Gemeinden unterteilen lässt. Die Hauptstadt der Region ist die Stadt Portalegre, die mit 22.368 Einwohnern in der gesamten Gemeinde und 14.318 im Stadtgebiet die zweitgrößte Stadt der Subregion ist, nach Elvas mit 14.438 Einwohnern. Sie grenzt im Norden an die Subregion Beira Baixa, im Osten an die Extremadura (Spanien), im Süden an die Subregion Alentejo Central, im Südwesten an die Subregion Lezíria do Tejo und im Nordwesten an die Subregion Médio Tejo.
